I have a number of AG doll patterns listed and I include the following in the description:
When purchasing more than one item, please wait for an adjusted invoice prior to making payment.
Some buyers will do this, while others don't, paying for each one separately.
As these go for very low $$, would I be within my rights to add the 30 cent PayPal fee to the additional 50 cents per pattern for shipping prior to remitting the buyer a refund? I would include an explanation in the note section of the PayPal refund? I'm sure buyers have no idea of the PayPal fees.

So, they are buying and paying each on individually? Like they are buying and paying. Then buying and paying again? I have to wonder if the ones who are doing this are being forced into IPR by eBay. How many are they usually buying at a time?
Do you have a rule set up for shipping on these items? You can do this for specific items but if they aren't adding to cart or are buying and paying each one, then not sure it would matter.
Depending on how many they buy and how much the refund would be, I doubt they would notice 30 cents missing from a shipping refund. In fact, I bet most of them aren't even expecting a refund on shipping at all.

And no, I don't have any combined shipping thing set up through eBay. I considered it once, but quit after trying to figure out how to do it.
If the profit margin wasn't so low, I wouldn't worry about it, but when selling items in the area of $2.99, fees really take a huge bite.

Yes. eBay has some buyers set up to buy only with IPR. Seller can't do anything about that. I've started shipping packages separately when a buyer buys them individually. The shopping cart has been on the site for years now. I'm tired of buyers not bothering to figure out how to work it. Not my problem. So far, no one has complained.

Like Val said, yes, eBay has randomly put some buyers into forced IPR for a number of years now. I would either ship separately or refund minus the PayPal fee. If you want to set up a shipping discount, I can post instructions on how to do it. You can save several of them and apply them only to certain listings during the listing process. It might help, it might not.

They are supposed to be able to, but there have long been random issues with IPR and use of the cart...sometimes it works, sometimes it won't allow them to request a total and makes them pay with full shipping on each item.

It drives me nuts when people don't use the cart, but then I had a regular buyer tell me that the cart doesn't always work, and a couple of other people who purchased and paid separately (I'm all IPR) write me afterwards to apologise for all the individual sales and say they couldn't get things to add in their cart and had to check out separately.
So it does happen, but I don't think it's as often as people seem to want to buy everything individually.
